The trend of troponin concentration suggests that del Nido cardioplegia has a better myocardial protective effect. Blood cardioplegia (BC), which is the mixture of the subject's oxygenated blood (80%) and a crystalloid solution (20%), is the most widely used cardioplegia type [ 1]. The del Nido solution was formulated by researchers from the University of Pittsburgh (Pittsburgh, PA, USA) in the early 1990s [ 5]. The del Nido cardioplegia (DNC), a non-glucose-based and calcium-free solution (four-parts crystalloid to one part whole blood), is a simple and cost-effective formulation that has been safely employed for decades in paediatric patients. 6, 7 It is generally used in a single-dose fashion (usually limited to 1000 ml) and subsequent doses are not necessary, except for extraordinarily long cross-clamp times (more than 2.5-3.0 hours).

The convenience offered by a single-dose cardioplegia strategy is the avoidance of interruption of the flow of surgery and, more importantly, a significant reduction in the cross-clamp time.

Different cardioplegic strategies have been evaluated. Del Nido cardioplegia was initially used in pediatric patients and has been expanding into adult cardiac centers over the last decade. Del Nido cardioplegia is routinely used in clinical practice both in paediatric and adult population in high volume centres. It could be a safe, effective and cheap myocardial protection strategy of universal implementation in cardiac surgery. Additional prospective studies are needed to confirm these data.
